scummvm - 2.6.1
Step-by-step reproduction of bug:
1) load attached save file
2) click verb "Open"
3) click on the "drawers" (bottom right of screen)
4) Mr. Epstein's portrait pops on screen and prevents opening of drawers, even though he is not on-screen. As an item from the drawers is needed, this puts the game into an unwinnable state. Compare to: https://youtu.be/heQTSkeL6y0?t=3634

Just to add on, after producing the bugged-state reported here, I replayed Serrated Scalpel (using DosBOX) from the beginning to the scene described above (which took ~ 20min), and was properly able to open the drawers. Which further corroborates that the behavior present in Scumm is indeed a bug.

I can confirm the bug (using the provided save), there's a workaround though.
Just try to leave, when Watson comes back and finishes talking you'll be able to open the drawers (even if Epstein is present).
This looks like a script bug, as if Epstein is set as "outside" when Watson finishes talking instead of when they actually move to the other room.
